The Opportunity Center strives to create opportunities that enable individuals with disabilities to reach their full potential in society. These opportunities are offered through a variety of client programs which can be tailored to each individual.

Assisted and Supervised Monday Through Friday

Unity Adult Day Center

Unity Adult Day Center offers services to adults with disabilities offering a safe and supported environment with Monday through Friday programs and activities. We have licensed and trained CNAs that specialize in dementia and memory care. Some of the activities we offer include:

  • Arts and Crafts

Our goal is to offer a safe and supported environment for all individuals with activities, community integration that allow self determination and individual growth. We do offer transportation to and from events. Drop in care is also available.

Outing

The primary objective of Unity Adult Day Care is to provide services to adults and frail elders who need enriched social or health-supportive experiences, or who may need assistance with activities of daily living (ADLs), supervision and/or protection. 

Services include supervision and training based on individual needs and desires. All programming takes into consideration individual differences in health and functioning, lifestyle, ethnicity, values, needs, interests, abilities and skills. Services may include personal services (eating, bathing, grooming, medication administration, supervision, and assistance in the use of adaptive equipment), transportation, and montessori activities designed to meet physical, social and leisure time needs. In addition, there are community outings scheduled on a regular basis including unique inter-generational and multi-ability programs. Unity offers a unique opportunity for participants to interact with local day care and school children through scheduled visits and activities.

Community Connections

The Community Connection Program is about giving adults with disabilities opportunities to broaden their horizons while supporting them as individuals, not just their needs. This is an evening and weekend engagement program open to all adults with disabilities, frail elders, and those with dementia. The goal of this program is to create a sense of belonging and combat loneliness.

Some of the activities we have done and continue to offer are:

  • Mini Golf
  • Movies and Shows
  • County Fairs
  • Swimming and Water Aerobics

Trained staff, 1 staff for every 4 client ratio, will guide the group but self discovery and determination will be encouraged and supported. Individuals will be able to choose which events they would like to participate in and can sign up online or in person. While some activities are free, there are activities that may incur fees.

Group Supported Employment Services

Group Supported Employment is a program offered through the Division of Vocational Rehabilitation to facilitate the employment of individuals with disabilities into community integrated work settings.

Services within this program include:

Transitional Youth Program

The Youth Transition Program is designed to assist persons with disabilities who are currently in high school, and are interested in transitioning into integrated work settings following their graduation. The program focuses on building functional skills that will be necessary for integrated employment. This is done through soft skills training in a job as well as community integration experiences.

This program includes a curriculum that focuses on such skills as: training on social skills, decision-making skills, employability, telephone skills, and computer skills. This also includes how to get along with coworkers, anger management, active/effective listening, problem solving, proper attire and hygiene, workplace safety, filling out job applications, developing resumes, interviewing, and many other day-to-day activities.  

Along with this program and its curriculum, youth also have the opportunity to go on tours of local businesses, complete job shadow opportunities, spend time volunteering in community settings, and receive training in job specific settings.

Real World Experiences

Building Full Lives

​The primary objective of the this program is to provide an avenue for achieving a full and meaningful life for adults living with a disability. The program encompasses necessary soft skills for employment, daily living skills and community integration methods to ensure that the individual will be successful both at their job and in their personal life outside of work.  

In this program, individuals with disabilities will learn every day skills needed to maintain a job and lead a meaningful life through ​both classroom and real world, community based experiences. Skills included in this curriculum include; accessing community resources, money management, boundaries and relationships, basic social skills, health and stamina, safe technology use, hygiene and appropriate dress, home care maintenance, nutrition and food preparation, self-advocacy, diversity/cultural awareness, public transportation use, and community safety.
